The main conceptual idea is that a Japan coastguard plane's crew misinterpreted a taxi instruction as clearance to enter the runway, leading to a fatal collision with a Japan Airlines jetliner. Despite the co-pilot correctly repeating the instruction to "taxi to holding point C5," the crew subsequently acted as if they had been given permission to proceed onto the runway, ultimately resulting in the accident. Evidence from flight voice recordings supports this misinterpretation.
